Self-Directed Services provides for people who are interested in self directing and managing their personalized services based on their individual needs and goals.
Self-Directed Services
Self-Directed Services operates as a "Co-Manager" with the individual and their circle of support acting as the "Employer of Record." The family is the "Managing Employer" and the "On-Site Supervisor." In this model, our Self-Directed Service provides a multitude of supports, including providing a compliant hiring process with requisite background checks, required training to staff, payroll and benefits (when applicable) and reimbursements based upon the individualized plan. Self-Directed Services also collects and reviews all necessary paperwork and documentation to ensure compliance with Medicaid/OPWDD payments.
Through this cooperative effort, individuals are able to control and implement their own budgets and realize their goals through community inclusion as outlined in their personal self-directed plan.
Program Eligibility
To be eligible for services, one must have a life-long disability which occurred before the age of 22 of which include diagnoses such as Intellectual Disabilities and other Developmental Disabilities, Cerebral Palsy, Epilepsy, Learning Disabilities, Autism, and other Neurological Impairments.
The primary diagnosis must be "Intellectually/Developmentally Disabled" as defined by OPWDD.
Cost: None to Medicaid eligible individuals.

- Be Waiver Services enrolled (Your Care Coordinator will assist in enrollment).
