Structured activities and supports provided in natural settings or agency facilities to promote independence, inclusion, choice, self-esteem and skill development.
In our Day Habilitation programs, each person receives an individually designed program of general assistance in accordance with their Life Plan. Day Habilitation helps individuals to succeed more effectively within their environment by developing services that are directed toward acquisition, retention, and/or improvement of skills in the following areas:
Services are based out in our community, as well as out of our ExploreABILITY Building.
Community Prevocational Services are supports and services that prepare an individual for employment that are job task oriented.
These are generally known as "soft skills" which include:
Generally, skill development services are provided in the community in volunteer settings.
To be eligible for services, one must have a life-long disability which occurred before the age of 22 of which include diagnoses such as Intellectual Disabilities and other Developmental Disabilities, Cerebral Palsy, Epilepsy, Learning Disabilities, Autism, and other Neurological Impairments.
The primary diagnosis must be "Intellectually/Developmentally Disabled" as defined by OPWDD.
Cost: None to Medicaid eligible individuals.
